Health Insurance in the Netherlands - What You Need to Do

At a glance: Dutch law requires you to hold Dutch basic health insurance (basisverzekering) within 4 months of starting work. You arrange this yourself - Toku does not enrol you. Coverage backdates to your start date, so don't delay. International or expat plans (Cigna, Aon, Allianz, OOM) do not count.

Your Responsibility as an Employee

Dutch law requires everyone who lives or works in the Netherlands to hold Dutch basic health insurance (basisverzekering). This is your personal responsibility to arrange - Toku does not select or enrol you in a health plan on your behalf.

You must enrol within 4 months of starting work or registering your address in the Netherlands. Coverage applies retroactively from your start date, so even if you delay, you will still owe premiums from day one.

What Counts as Valid Insurance?

Only a Dutch basisverzekering from a Dutch-licensed insurer satisfies this requirement. International or expat plans (e.g., Aon, Cigna, OOM, Allianz) do not qualify, even if they provide health coverage in the Netherlands. If you currently hold one of these plans, you are considered uninsured by the Dutch government.

Step-by-Step: How to Enrol

  1. Confirm you have a BSN: You must be registered with your municipality and have a Burgerservicenummer (BSN) before you can take out Dutch basic insurance.
  2. Choose a Dutch insurer: Select a basisverzekering from any Dutch-licensed provider. Visit hetcak.nl for a list of approved insurers.
  3. Enrol promptly: Do this as soon as possible after your start date. Your policy should be backdated to the date your obligation began.
  4. Notify Toku: Once enrolled, email [email protected] with your premium details (see below for the exact information we need).

If You Receive a Letter from the CAK

The CAK (Centraal Administratie Kantoor) is the Dutch government body that monitors health insurance compliance. If you receive a letter from them stating you are uninsured:

  • Do not ignore it: You have 3 months from the date of the letter to take action.

  • Choose Action A (get Dutch basic health insurance) if you are employed in the Netherlands - this applies to you.

  • If you believe you may be exempt (e.g., working for a foreign employer under specific conditions), you may apply for a Wlz assessment through the SVB at svb.nl — but given your employment with Toku's WorkCo entity in the Netherlands, exemption is unlikely. What happens if you don't act:

  • Fines: €529.74 per instance (2026 rate) - your personal responsibility.

  • Forced enrolment: The CAK will eventually assign you to an insurer and charge an administrative premium of €172.70/month (2026 rate), which Toku is legally required to withhold from your salary and remit to the CAK on your behalf.

How to Notify Toku of Your Premium

Once you have enrolled in a Dutch basic health insurance plan, please email [email protected] with:

  • Your full name and employee ID
  • The name of your Dutch health insurer
  • Your monthly premium amount
  • The effective start date of your policy This allows Toku to keep your payroll records accurate and ensures compliance with Dutch employer obligations under the Zorgverzekeringswet (Zvw).

Pension in the Netherlands

Toku provides a mandatory occupational pension plan in the Netherlands in line with local regulations. This plan supports employees with retirement income, financial protection for dependents, and continued pension accrual in case of long-term disability.

You can manage your pension through the Centraal Beheer employee portal, where you can:

  • View your current pension balance

  • Set up or change voluntary contributions Login details are sent directly by Centraal Beheer.

Employees are automatically enrolled in the pension plan from their start date. No action is required unless you choose to make voluntary contributions or add optional coverage.

Old Age Pension (Retirement)

  • You and Toku contribute monthly to your pension account

  • Your pension value grows through:

  • At retirement, your accumulated balance is used to purchase:
    • A lifelong old age pension
    • A partner pension You may choose to retire earlier or later than the target age of 68, subject to pension rules.

Partner & Survivor Benefits

Partner Pension (Before Retirement)

  • 30% of pensionable salary
  • Paid immediately upon death
  • Risk-based coverage (applies only while employed)

Orphan’s Pension

  • 6% of pensionable salary
  • Paid to eligible children up to age 25

Voluntary Partner Pension (ANW-gap Coverage)

  • Optional additional insurance
  • Paid by the employee
  • Provides an additional 2,226€20,022 (2026) gross per year
  • Designed to supplement state survivor benefits

Partner Pension (After Retirement)

  • 70% of the old age pension amount chosen at retirement

Disability Coverage (Waiver of Premium)

If you become long-term disabled as determined by the Dutch social security authority:

  • Your pension contributions continue
  • No cost to you
  • Contributions remain based on your insured salary This ensures your retirement savings continue to grow even if you are unable to work.
