A guide for employees on Japan’s statutory annual health check

Overview
In Japan, an annual health checkup is a legal requirement for all employees. This guide explains what the checkup is, what is required of you, and how to arrange it.
The health checkup is not covered by health insurance. As the employer of record, Toku covers the cost of the checkup on your behalf, in line with the Industrial Safety and Health Act (労働安全衛生法), which makes the annual checkup mandatory.
Ippan Kenshin: General Health Checkup
The checkup must be taken once within the health insurance association’s designated period, which runs from April through March of the following year.
The General Health Checkup assesses your overall health and includes:
- Basic examination (height, weight, BMI, abdominal girth, vision, hearing, blood pressure, and a brief doctor’s interview)
- Urine test
- Chest X-ray
- Electrocardiogram (ECG)
- Blood test Please note: If your results indicate that further medical consultation or treatment is needed, you should contact the clinic or doctor directly to arrange it. The cost of any additional examination or treatment beyond the standard checkup is not covered by the company.
Cost & coverage
- The General Health Checkup is covered up to a maximum of JPY 60,000 per person.
- A bilingual service is available, with the result report provided in English at Tokyo Midtown Clinic.
- The result report can be issued in either Japanese or English. An additional charge applies if you request the report in both languages.
How to arrange your checkup
Option 1 - Toku’s contracted clinic (recommended)
You can visit Tokyo Midtown Clinic, with which Toku has a partnership. You do not need to pay the checkup fee yourself, as Toku settles payment directly with the clinic.
Option 2 - A clinic of your choice
If you prefer to visit another clinic or hospital, the fee will be reimbursed through payroll up to a cap of JPY 15,000. Please be sure to keep and submit the original receipt to claim reimbursement.

Important notes
- Remember to bring your health insurance card when you visit the clinic for your checkup.
- If the fee exceeds the coverage amount, you must pay the difference at the clinic.
